>> AirAsia has flights daily between Honolulu and Kuala Lumpur, Malaysia. It increased service from three times a week in August. The information accompanying a story on Page 8 of Tuesday’s Live Well section was inaccurate.

>> Ursula Retherford, a resident of Kailua, said, “My foremost hope for this plaque is that it will always inspire people to believe in the power within them and band together to defend what is dear to them.” Her last name was misspelled and she was misquoted in a story on Page B1 Friday on the plaque commemorating efforts to preserve the Ka Iwi coastline.

>> In 19 cases, DNA identified in a victim’s rape kit matched that taken from another crime scene. That information wasn’t clearly reported in a story on Page A1 Nov. 5 on the backlog for testing of rape test kits in Hawaii.

>> Patsy Mink was first elected to Congress in 1965. The year was incorrect in a Lee Cataluna column on Page B1 Friday.
