The state Department of Taxation put out an alert today for Hawaii island taxpayers that their tax documents may have been lost.

A courier bag containing the documents was discovered missing from a Security Courier Service vehicle on Oct. 4, department officials said. The company is contracted by the tax department as their courier service.

Officials said about 75 taxpayers may be affected. The documents lost have been narrowed down to the following:

Business License Forms (Form BB-1) and Cancellation Forms (Forms GEW-TA-RV-1) that were mailed or dropped off between September 24-October 1, 2012 at the department’s Kona office.Tax documents mailed on or about October 1-3, 2012, or dropped off on or about October 3-4, 2012 at the department’s Hilo office. The documents include business license applications, tax returns (GET and income tax) with checks, and payment vouchers.
