The Honolulu Medical Examiner’s Office has identified the 54-year-old woman who was fatally struck by a vehicle at the Mililani Shopping Center as Donna A. Miguel of Ewa Beach.

The cause of death is pending.

Police said a 74-year-old man was operating a vehicle at a drive-thru of an automated teller machine at about 11:20 a.m. Tuesday when, for unknown reasons, he lost control of the car.

The car reversed and struck another vehicle and a woman later identified as Miguel.

Police said she was standing next to an open door of the vehicle at the time of the crash.

Miguel sustained a head injury and was taken in serious condition to the Queen’s Medical Center where she later died.

Police said speed, alcohol and drugs do not appear to be factors in the crash at this time.

The investigation is ongoing.
