The 3-year-old daughter of former University of Hawaii All-American volleyball player Kim Willoughby was pronounced dead Tuesday after the child slipped in the bathtub and hit her head, El Vocero de Puerto Rico newspaper reported.

According to the police report, Willoughby was looking for a towel when her daughter slipped in the tub. CPR was performed but the toddler was pronounced dead at Centro Medico de Mayaguez, the hospital in the city where Willoughby plays professionally for Las Indias.

Willoughby was a three-time All-American for the Rainbow Wahine (2001-03) and the 2003 AVCA National Player of the Year as a senior. She was a member of the U.S. Olympic team that took the silver medal at the 2008 Beijing Olympics.
